A national initiative to improve cardiovascular health for all Americans
About us
Heart disease, stroke, and other cardiovascular diseases (CVDs) affect more than 1.5 million Americans each year. Heart disease is the nation’s number one killer among both men and women and the leading cause of health disparities across the country. And while death rates have been declining steadily for the last 40 years, this trend appears to be stalling. To address this issue, the U.S. Department of Health and Human Services (HHS) established Million Hearts®, a national initiative co-led by the Centers for Disease Control and Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS). Alongside 120 official partners and 20 federal agencies, Million Hearts® works to align national CVD prevention efforts around evidence-based public health and clinical strategies to prevent 1 million cardiovascular events within 5 years. Black adults have twice the rate of death from hypertension-related disorders compared to White adults. This month, commit to helping improve equitable health care by teaching your patients to monitor their own blood pressure at home with self-measured blood pressure monitoring (SMBP). Coupled with clinical guidance, SMBP can help reduce hypertension and improve quality of care! https://bit.ly/3GIKYcL
